OBJECTIVE: The internet has become an important element in people's lives. The increasing use of smartphones and other electronic devices has promoted an increase in digital interactions, resulting in significant problems in the field of mental health. However, one of the implications of excessive internet use is impaired sleep quality, especially among users who use the internet before bed. METHODS: We searched the PubMed, Web of Science and PsycINFO databases to identify articles that addressed the association between excessive internet use and sleep quality. Studies in which participants had a previous diagnosis of insomnia or used psychoactive substances were excluded. RESULTS: The initial search resulted in a total of 3269 articles, 25 of which met the inclusion criteria. The results suggested an association between excessive internet use and sleep quality. CONCLUSION: Excessive internet use significantly compromises sleep quality, directly affecting users' mental and physical health. This study highlights the importance of strategies that promote digital education to raise awareness about the risks of excessive use of screens. BACKGROUND: Digital connectivity is essential in modern work environments, enhancing productivity and communication. However, its rapid expansion post-COVID-19 raises concerns about burnout, digital fatigue, and work-related stress. OBJECTIVE: This PRISMA-based systematic review examines the benefits and challenges of digital work, assessing its impact on occupational health and proposing mitigation strategies. METHODS: A systematic search of PubMed, Google Scholar, Scopus, and Cochrane Library identified 40 peer-reviewed studies published since 2020, focusing on digital connectivity, remote work, and employee well-being. Studies on purely technological aspects were excluded. RESULTS: While digital tools improve efficiency and flexibility, they also increase workload, cognitive overload, and stress. Prolonged screen exposure contributes to mental exhaustion and sleep disturbances. Limited digital infrastructure further exacerbates productivity barriers. CONCLUSIONS: Digital connectivity offers both opportunities and risks. Organizations should implement structured policies such as offline hours, digital detox initiatives, and mental health support to sustain productivity and well-being. Future research should explore sector-specific interventions and long-term impacts of digital work practices.

The impact of social media has been significant on various aspects of life, particularly mental health. Growing concerns about the adverse effects of social media use have prompted the exploration of experimental interventions, defined as digital detox interventions. However, it remains unclear whether digital detox interventions are effective for mental health outcomes. The aim of this study was to provide comprehensive insights into the effects of digital detox interventions on various mental health outcomes, including depression, life satisfaction, stress, and mental well-being. Following the PRISMA guidelines, systematic searches were carried out in online databases, including PubMed and ScienceDirect, within the publication range of 2013 and 2023. A total of 2578 titles and abstracts were screened, and 10 studies were included in the analysis. A risk of bias assessment was conducted using RoB 2.0 and the Newcastle-Ottawa scale, while statistical analysis was conducted using RevMan 5.4.1. Our data indicated a significant effect of digital detox in mitigating depression with the standardized mean difference (SMD: -0.29; 95%CI: -0.51, -0.07, p=0.01). No statistically significant effects were discerned in terms of life satisfaction (SMD: 0.20; 95%CI: -0.12, 0.52, p=0.23), stress (SMD: -0.31; 95%CI: -0.83, 0.21, p=0.24), and overall mental well-being (SMD: 0.04; 95%CI: -0.54, 0.62, p=0.90). These data underscore the nuanced and selective influence of digital detox on distinct facets of mental health. In conclusion, digital detox interventions significantly reduce depressive symptoms, suggesting that intentional reduction or cessation of digital engagement may help alleviate contributing factors. However, no statistically significant effects were observed in mental well-being, life satisfaction, and stress. This discrepancy may be due to the complex nature of these constructs, involving various factors beyond the scope of digital detox interventions. © 2024 The Author(s).

OBJECTIVE: To determine the association between genetic factors and molar-incisor hypomineralisation (MIH) and/or hypomineralised second primary molars by means of a systematic review. DESIGN: A search was performed in Medline-PubMed, Scopus, Embase and Web of Science databases; manual search and search in gray literature were also performed. Selection of articles was performed independently by two researchers. A third examiner was involved in cases of disagreement. Data extraction was performed using an Excel® spreadsheet and independent analysis was performed for each outcome. RESULTS: Sixteen studies were included. There was an association between MIH and genetic variants related to amelogenesis, immune response, xenobiotic detoxification and other genes. Moreover, interactions between amelogenesis and immune response genes, and SNPs in the aquaporin gene and vitamin D receptors were associated with MIH. Greater agreement of MIH was found in pairs of monozygotic twins than dizygotic twins. The heritability of MIH was 20 %. Hypomineralised second primary molars was associated with SNPs in the hypoxia-related HIF-1 gene and methylation in genes related to amelogenesis. CONCLUSION: With very low or low certainty of evidence, an association was observed between MIH and SNPs in genes associated with amelogenesis, immune response, xenobiotic detox and ion transport. Interactions between genes related to amelogenesis and immune response as well as aquaporin genes were associated to MIH. With very low certainty of evidence, hypomineralised second primary molars was associated to a hypoxia-related gene and to methylation in genes related to amelogenesis. Moreover, higher agreement of MIH in pairs of monozygotic twins than dizygotic twins was observed.

BACKGROUND: Esophageal carcinoma is the third most common gastrointestinal malignancy worldwide and is largely unresponsive to therapy. African-Americans have an increased risk for esophageal squamous cell carcinoma (ESCC), the subtype that shows marked variation in geographic frequency. The molecular architecture of African-American ESCC is still poorly understood. It is unclear why African-American ESCC is more aggressive and the survival rate in these patients is worse than those of other ethnic groups. METHODS: To begin to define genetic alterations that occur in African-American ESCC we conducted microarray expression profiling in pairs of esophageal squamous cell tumors and matched control tissues. RESULTS: We found significant dysregulation of genes encoding drug-metabolizing enzymes and stress response components of the NRF2- mediated oxidative damage pathway, potentially representing key genes in African-American esophageal squamous carcinogenesis. Loss of activity of drug metabolizing enzymes would confer increased sensitivity of esophageal cells to xenobiotics, such as alcohol and tobacco smoke, and may account for the high incidence and aggressiveness of ESCC in this ethnic group. To determine whether certain genes are uniquely altered in African-American ESCC we performed a meta-analysis of ESCC expression profiles in our African-American samples and those of several Asian samples. Down-regulation of TP53 pathway components represented the most common feature in ESCC of all ethnic groups. Importantly, this analysis revealed a potential distinctive molecular underpinning of African-American ESCC, that is, a widespread and prominent involvement of the NRF2 pathway. CONCLUSION: Taken together, these findings highlight the remarkable interplay of genetic and environmental factors in the pathogenesis of African-American ESCC.

Although completion of detoxification (detox) and a successful transition from detox to substance use disorder (SUD) treatment and/or mutual-help groups are associated with better SUD outcomes, many patients do not complete detox or do not receive SUD care following detox. The purpose of this structured evidence review, summarizing data extraction on a yield of 26 articles, is to identify patient, program, and system factors associated with the outcomes of completion of alcohol detox and successful transitions from alcohol detox to SUD treatment and mutual-help group participation. The review found wide variability among studies in the rates at which patients complete a detox episode (45 to 95%) and enter SUD treatment or mutual-help groups after detox (14 to 92%). Within program factors, behavioral practices that contribute to both detox completion and transitioning to SUD care after detox entail involving the patient's family and utilizing motivational-based approaches. Such practices should be targeted at younger patients, who are less likely to complete detox. Although more studies using a randomized controlled trial design are needed, the evidence suggests that barriers to detox completion and transition to SUD care can be overcome to improve patient outcomes. INTRODUCTION: Dependence on opioids is a multifactorial condition involving genetic and psychosocial factors. There are three stages to treating opioid dependence. Stabilisation is usually by opioid substitution treatments, and aims to ensure that the drug use becomes independent of mental state (such as craving and mood) and independent of circumstances (such as finance and physical location). The next stage is to withdraw (detox) from opioids. The final stage is relapse prevention. This treatment process contributes to recovery of the individual, which also includes improved overall health and wellbeing, as well as engagement in society. METHODS AND OUTCOMES: We conducted a systematic review and aimed to answer the following clinical questions: What are the effects of drug treatments for stabilisation (maintenance) in people with opioid dependence? What are the effects of drug treatments for withdrawal in people with opioid dependence? What are the effects of drug treatments for relapse prevention in people with opioid dependence? We searched: Medline, Embase, The Cochrane Library, and other important databases up to March 2011 (Clinical Evidence reviews are updated periodically; please check our website for the most up-to-date version of this review). We included harms alerts from relevant organisations such as the US Food and Drug Administration (FDA) and the UK Medicines and Healthcare products Regulatory Agency (MHRA). RESULTS: We found 26 systematic reviews, RCTs, or observational studies that met our inclusion criteria. We performed a GRADE evaluation of the quality of evidence for interventions. CONCLUSIONS: In this systematic review, we present information relating to the effectiveness and safety of the following interventions: buprenorphine; clonidine; lofexidine; methadone; naltrexone; and ultra-rapid withdrawal regimens.

We searched: Medline, Embase, The Cochrane Library and other important databases up to June 2006 (Clinical Evidence reviews are updated periodically, please check our website for the most up-to-date version of this review). We included harms alerts from relevant organisations such as the US Food and Drug Administration (FDA) and the UK Medicines and Healthcare products Regulatory Agency (MHRA). RESULTS: We found 21 systematic reviews, RCTs, or observational studies that met our inclusion criteria. We performed a GRADE evaluation of the quality of evidence for interventions. CONCLUSIONS: In this systematic review we present information relating to the effectiveness and safety of the following interventions: buprenorphine, clonidine, lofexidine, methadone, naltrexone, and ultra-rapid withdrawal.

PURPOSE OF RESEARCH: Application of psychedelics including ketamine may be important adjuncts in the treatment of alcohol and opioid use disorders. Most patients with alcohol or opioid use disorder (AUD/OUD) may present to the emergency department (ED) seeking care both for substance use and associated complaints. Understanding safety and preliminary efficacy of ketamine and other psychedelics to improve the care and potentially facilitate linkage to long term care for AUD/OUD in the ED is of high importance. Here, we describe our experience conducting rigorous double blind placebo controlled trials to evaluate safety of ketamine in the ED for AUD/OUD and the challenges in conducting this research among this important population. PRINCIPAL RESULTS: We conducted two studies to evaluate the safety of a single dose ketamine infusion for individuals seeking detox with AUD and those seeking psychiatric care who had OUD. Studies were conducted at an urban community and academic ED with variable pharmacy capabilities. Over the study period study 1 (AUD) screened N = 180 individuals for the study of which N = 3 were preliminarily eligible. None were able to pass full eligibility criteria resulting in no enrollments. Study 2 (OUD) screened N = 234 ED visits were screen of which N = 8 were eligible and N = 4 consented. All four participants were successfully randomized and treated with ketamine or placebo. We were unable to conduct follow up in any of the participants. MAJOR CONCLUSIONS: Enrollment of ED patients with AUD/OUD for ketamine trials in the ED is difficult and fraught with multiple challenges. Even with enrollment in the ED, follow up can be a significant challenge. This reinforces the need to consider innovative strategies to conduct ketamine and other psychedelic related trials among ED patients with AUD/OUD.
