BACKGROUND AND AIMS: Emblica Officinalis (Amla) is a plant often utilized in traditional medicine due to its purported anti-inflammatory, antioxidant, hypoglycemic, and hypolipidemic properties. However, current evidence regarding its potential for preventing and treating metabolic abnormalities associated with chronic diseases remains unclear. METHODS: This systematic review and meta-analysis aimed to examine the effects of Amla supplementation on lipid profile, glucose, and C-reactive protein (CRP) concentrations in adults. We completed a systematic search (current as of December 2022) of all available randomized controlled trials (RCTs) in the database including ISI Web of Science, PubMed, Scopus, and Embase. Any effect's mean difference (MD) was calculated using a random-effects model. Weighted mean difference (WMD) and 95% confidence intervals (CIs) were calculated also calculated using a random-effects model. RESULTS: Five RTCs were included in the meta-analysis. Following Amla supplementation, pooled results showed a significant reduction in CRP (p = 0.002), fasting blood glucose (FBG) (p < 0.001), low-density lipoprotein cholesterol (LDL-c) (p < 0.001), total cholesterol (TC) (p < 0.001), and serum triglyceride (TG) (p < 0.001) concentrations as well as an increase in high-density lipoprotein cholesterol (HDL-c) (p < 0.001). The baseline concentration of biochemical indicators was used for subgroup analysis. CONCLUSION: Amla supplementation shows promise for improving metabolic parameters in adults. In general, the populations included in the analysis were generally 40-58 years with an average BMI of 25.5 and a length of intervention ranging from 3 to 12 weeks. Thus additional investigations are warranted to confirm and expand the findings presented herein.

BACKGROUND: Cough during endobronchial ultrasound (EBUS) is common and can reduce patient comfort, prolong procedures, and compromise diagnostic yield. While dextromethorphan has demonstrated antitussive efficacy, concerns remain regarding side effects and availability. Emblica officinalis (Amla) is a traditional herbal medicine with reported antitussive properties, but human evidence is lacking. OBJECTIVE: This study evaluated the efficacy of E. officinalis compared with dextromethorphan and their combination as premedication for cough suppression in patients undergoing advanced EBUS. METHODS: In this randomized, double-blind, placebo-controlled trial, 118 patients undergoing EBUS were allocated to placebo, dextromethorphan 60 mg, E. officinalis extract 600 mg, or their combination. Cough events were recorded throughout the procedure. The primary outcome was total cough count. Secondary outcomes included oxygen desaturation, sedative/analgesic requirements, procedure difficulty, and patient-reported discomfort. RESULTS: Median cough counts were 61.0 (IQR 26.3-90.8) in placebo, 20.0 (5.0-27.0) with dextromethorphan, 27.0 (14.0-40.0) with E. officinalis, and 25.5 (15.3-29.0) with the combination. Kruskal-Wallis testing showed significant group differences (p = 0.019, rank η²=0.061). All active treatments reduced cough versus placebo (p < 0.05), with no differences among active arms. E. officinalis reduced oxygen desaturation, while dextromethorphan lowered sedative requirements. No adverse events occurred. CONCLUSION: E. officinalis significantly reduced cough during EBUS, with efficacy comparable to dextromethorphan and an excellent safety profile. These findings suggest E. officinalis may serve as a safe, cost-effective alternative premedication for cough suppression in advanced bronchoscopy.

Background: Exercise and nutritional interventions are often recommended to help manage risk related to metabolic syndrome (MetSyn). The co-ingestion of Phyllanthus emblica (PE) with trivalent chromium (Cr) has been purported to improve the bioavailability of chromium and enhance endothelial function, reduce platelet aggregation, and help manage blood glucose as well as lipid levels. Shilajit (SJ) has been reported to have anti-inflammatory, adaptogenic, immunomodulatory, and lipid-lowering properties. This study evaluated whether dietary supplementation with Cr, PE, and SJ, or PE alone, during an exercise and diet intervention may help individuals with risk factors to MetSyn experience greater benefits. Methods: In total, 166 sedentary men and women with at least two markers of metabolic syndrome participated in a randomized, placebo-controlled, parallel-arm, and repeated-measure intervention study, of which 109 completed the study (48.6 ± 10 yrs., 34.2 ± 6 kg/m2, 41.3 ± 7% fat). All volunteers participated in a 12-week exercise program (supervised resistance and endurance exercise 3 days/week with walking 10,000 steps/day on non-training days) and were instructed to reduce energy intake by -5 kcals/kg/d. Participants were matched by age, sex, BMI, and body mass for the double-blind and randomized supplementation of a placebo (PLA), 500 mg of PE (PE-500), 1000 mg/d of PE (PE-1000), 400 µg of trivalent chromium (Cr) with 6 mg of PE and 6 mg of SJ (Cr-400), or 800 µg of trivalent chromium with 12 mg of PE and 12 mg of SJ (Cr-800) once a day for 12 weeks. Data were obtained at 0, 6, and 12 weeks of supplementation, and analyzed using general linear model multivariate and univariate analyses with repeated measures, pairwise comparisons, and mean changes from the baseline with 95% confidence intervals (CIs). Results: Compared to PLA responses, there was some evidence (p < 0.05 or approaching significance, p > 0.05 to p < 0.10) that PE and/or Cr with PE and SJ supplementation improved pulse wave velocity, flow-mediated dilation, platelet aggregation, insulin sensitivity, and blood lipid profiles while promoting more optimal changes in body composition, strength, and

OBJECTIVE: To assess the efficacy of Phyllanthus emblica extract in alleviating halitosis and reducing the inflammatory response to halitosis-related bacteria. METHODOLOGY: This investigation, using Phyllanthus emblica fruit extract (PE), involved four aspects. First, we evaluated the effect on growth and aggregation of halitosis-related bacteria, including Fusobacterium nucleatum, Porphyromonas gingivalis, and Solobacterium moorei, using a microdilution assay and scanning electron microscopy. Second, volatile sulfur compound (VSC) levels were measured on individuals with halitosis in randomized short-term (26 participants) and double-blind randomized long-term trials (18 participants in each group) after rinsing with PE for 3, 6, and 12 h, and 28 days. Third, we analyzed pro-inflammatory cytokine expression in TR146 cells using quantitative real-time PCR and enzyme-linked immunosorbent assays. Lastly, we assessed pro-inflammatory cytokine secretion and Toll-like receptor (TLR) 2 mRNA expression via the same experimental methods in a three-dimensional oral mucosal epithelial model (3D OMEM). RESULTS: PE extract dose-dependently inhibited the growth of F. nucleatum (50% inhibition concentration [IC50]=0.079%), P. gingivalis (IC50=0.65%), and S. moorei (IC50=0.07%) and effectively prevented bacterial aggregation. Furthermore, VSC contents decreased significantly at 3, 6, and 12 h after rinsing with 5% PE compared with those in the control. Long-term use of mouthwash containing 5% PE for 28 days led to a significant decrease in VSC contents. PE attenuated the F. nucleatum- or P. gingivalis-stimulated mRNA expression and protein release of interleukin (IL)-6 and IL-8 in TR146 cells. It also suppressed IL-8 and prostaglandin E2 secretion and TLR2 mRNA expression in F. nucleatum-induced OMEMs. CONCLUSION: Our findings support the use of PE in oral care products to alleviate halitosis and it may reduce inflammation.

BACKGROUND: Gingivitis is frequently painless, rarely causes spontaneous bleeding and is manifested by minor clinical changes. Therefore, most patients are unaware of the disease or do not seek treatment, as it is asymptomatic. Several methods for removing microbial plaque have been proposed, including mechanical and chemical ones. Amla or Indian gooseberry is a medicinal herb; its secondary metabolites, such as phenolic acid, flavonoids and terpenoids, can be used to preferentially reduce metal ions and form nanoparticles (NPs). Green synthesis with the use of the amla seed extract is a unique approach for the production of graphene oxide (GO)-silver (Ag) nanocomposite mouthwash. OBJECTIVES: The aim of the present study was to prepare an amla seed-mediated GO-Ag nanocomposite mouthwash, and to assess its antibacterial and anti-inflammatory efficacy in plaque-induced gingivitis. MATERIAL AND METHODS: The present double-blind randomized controlled trial was conducted among 30 gingivitis patients. The patients were randomly allocated into 2 groups based on the intervention: group A (n = 15; nanocomposite mouthwash); and group B - control (n = 15; 0.2% chlorhexidine (CHX) mouthwash). Clinical parameters, including the plaque index (PI), the gingival index (GI), a microbiological parameter - colony forming units (CFUs), and a biochemical parameter - the C-reactive protein (CRP) level in gingival crevicular fluid (GCF), were assessed at baseline and at 15 days. RESULTS: The study results showed statistically significant differences in the mean PI and GI scores, and the CRP levels in the post-intervention period as compared to baseline in both groups. After the intervention period of 15 days, there were statistically significant differences between the 2 study groups in terms of mean PI and GI scores, and CRP levels. CONCLUSIONS: The amla seed-mediated GO-Ag nanocomposite mouthwash efficiently reduced plaque, gingival inflammation and CFUs among patients with plaque-induced gingivitis, but was not equivalent to the CHX mouthwash.

The intervention group received ten cc Amla syrup thrice a day for 12 weeks. The second group received a placebo with the same dose and duration. Hair growth parameters were analyzed using TrichoScan before and after 12 weeks of intervention. Physician and patient satisfaction were assessed using the CGI-I and PGI-I questionnaires, respectively. RESULTS: Twenty-seven participants in the intervention group and 25 in the placebo group completed the trial. Based on our findings, the anagen-to-telogen ratio increased significantly in the intervention group compared with the group who received placebo (F = 10.4, P = 0.002). Physician and patient satisfaction increased in the amla group compared with placebo at 12th weeks of intervention (P<0.001), (P<0.001). The formula had no remarkable side effects. Only one case of mild constipation was reported in one of the participants after one month of consuming Amla syrup. CONCLUSION: The results of this study demonstrated that Amla syrup could help treat androgenic hair loss in women and increase the anagen phase. Further studies are needed to evaluate this potential treatment for FAGA.

The present study aimed to optimize a multi-herbal combination exerting the greatest antioxidant property using statistical method for anti-skin aging application as well as to elucidate its in vivo safety and anti-skin aging potential. The multi-herbal combination was optimized using a two-level, full factorial approach by exploring the correlation between the concentrations (0-3%w/v) of three extracts from Centella asiatica (CA), Momordica cochinchinensis (MA), Phyllanthus emblica (EM). An anti-skin aging emulsion containing the optimized combination was then developed and evaluated for its physicochemical characteristics with its stability under storage conditions. The in vivo anti-skin aging potential of the emulsion was subsequently investigated among 60 women in a randomized, double-blind, placebo-controlled study. Skin hydration, elasticity and wrinkles at eye and cheek areas were measured at baseline, after 30 and 60 days of application. Before performance testing, in vivo skin irritation was evaluated using the patch test and homogeneity between groups was also statistically analyzed. According to the model describing the significant main effects of each extract and interaction effects between extracts on percent inhibition against DPPH radicals, the best multi-herbal combination consisted of 3%w/v EM and 3%w/v CA. The developed emulsion containing the combination presented smooth soft texture with good stability in terms of physical characteristics and biological property. Regarding the clinical study, no skin erythema and edema was reported among in all volunteers. After 60 days of application, significantly improved skin hydration, elasticity and wrinkles were observed in the test group. In addition, significantly reduced wrinkles were observed after 60 days in both skin areas of the test group. The anti-skin aging emulsion containing this optimized combination exhibited good safety and performance. Ultimately, this product comprises an effective anti-skin aging formulation for applications. © 2023. The Author(s).

OBJECTIVE: This randomized, double-blind, controlled trial (RCT) aimed to evaluate the effect of Phyllanthus Emblica (Amla) as an add-on therapy on COVID-19_ related biomarkers and clinical outcomes in COVID-19 patients. METHODS: In this RCT, sixty-one patients were randomly assigned into two arms [the intervention (n=31) and control arms (n=30)]. The effect of Amla on diagnostic Reverse-transcription Polymerase Chain Reaction (RT-PCR) test results between the first and the last days of the study, the length of stay (LOS) in hospital, the percentage of lung involvement on CT scans, changes in the clinical symptoms, and the laboratory markers were assessed. RESULTS: The two study groups had similar baseline demographics and characteristics in terms of medical history. The mean of LOS in the intervention arm (4.44 days) was significantly shorter than in the control arm (7.18 days, P < 0.001); RT-PCR results were not significantly different between the two arms (P = 0.07). All clinical variables decreased over time in the two groups (P < 0.001). However, the difference between the two groups in terms of fever (P = 0.004), severity of cough (P = 0.001), shortness of breath (P = 0.004), and myalgia (P = 0.005) were significant, but this intergroup comparison was not significant with regard to respiratory rate (P = 0.29), severity of chills (P = 0.06), sore throat (P = 0.22), and weakness (P = 0.12). Out of the eight evaluated para-clinical variables, three variables showed significant improvement in the intervention arm, including the mean increase in oxygen saturation (SpO2) level (P < 0.001), the reduction in the mean percentage of lung involvement on CT (P < 0.001), and the improvement in C-reactive protein test results (P < 0.001). CONCLUSION: Organic herbal Amla tea cannot significantly affect the RT-PCR results and or degree of lung involvement. Nevertheless, it showed an ameliorative effect on the severity of clinical signs and CRP levels. Also, Amla tea may shorten the recovery times of symptoms and LOS in COVID-19 patients. In recent years, polysaccharides with potential anticancer activity have attracted widespread attention. In this study, a homogeneous polysaccharide fraction, PEP-3 (102.35 kDa), was extracted and purified from the fruit of Phyllanthus emblica L. Structural analysis revealed that its backbone consists of methylated and unmethylated α-1,4-GalpA residues at the C-6 position. O-2 at α-2,4-Rhap-1→ participates in backbone extension, while O-4 serves as a branch point for the Galp and Araf side chains. Through machine learning-based target screening, HSP90 was identified as the core therapeutic target of PEP-3 for stomach adenocarcinoma (STAD) treatment and an important biomarker for diagnosis and prognosis. Mechanistic studies showed that PEP-3 inhibits HSP90 activity, leading to AKT degradation and activation of the Caspase cascade. In vitro experiments showed that the combination of PEP-3 and 5-fluorouracil (5-Fu) (PEP-3 concentration 400 μg/mL, 5-Fu concentration 3.13 μg/mL) exhibited a considerable synergistic effect, markedly enhancing apoptosis, inducing cell cycle arrest, promoting ROS and Ca2+ overload, and aggravating mitochondrial damage. These results indicate that PEP-3 enhances the chemosensitivity of 5-Fu by targeting HSP90 and inducing apoptosis through multiple pathways, while simultaneously reducing the toxic side effects of 5-Fu. This highlights its potential as a natural sensitizer for the treatment of STAD.
