OBJECTIVE: Nonalcoholic fatty liver disease (NAFLD) is the most prevalent chronic liver disease in the world. The pathogenesis of NAFLD is complex and multifactorial. Clinical studies have shown that alterations in the gut microbiota play a key role in NAFLD. The purpose of this study was to analyze the effect of probiotic supplementation on the treatment of NAFLD patients based on various indicators. METHODS: We conducted a meta-analysis investigating the relationship between NAFLD and probiotic supplementation. Embase, PubMed, and Web of Science databases were searched by computer, and then, eligible studies were identified. Finally, a total of high-quality randomized controlled trials were selected involving 1403 participants. Meta-analysis was performed using the RevMan 5.3 software which was systematically searched for works published through Dec. 1, 2021, in the present study. RESULTS: The meta-analysis results showed that the probiotics supplementation improved hepatocyte injury and significantly reduced the level of ALT (P = 0.00001), AST (P = 0.0009), GGT (P = 0.04), TG (P = 0.01), LDL-C (P = 0.0005), HDL-C (P = 0.0002), insulin (P = 0.003), IR (P = 0.03), BMI (P = 0.03), TNF-α (P = 0.03), and CRP (P = 0.02), respectively, in NAFLD patients. CONCLUSION: The present study suggests that probiotics therapy may improve liver enzyme levels, regulated lipid metabolism, reduced insulin resistance, and improved inflammation in NAFLD patients. It supports the potential role of probiotics supplementation in the treatment of NAFLD.

Gut health though is not well defined the role of gastrointestinal tract is vital if an animal must perform well. Apart from digestion, secretion, and absorption gut is harbored with consortium of microbiota which plays a key role in one's health. Enzymes, one of the alternatives for antibiotics with beneficial effects on digestion and consistency of food and its effect on gut health. The effect of enzyme supplementation on gut health is not well established and the objective of this meta-analysis is to investigate if the enzyme supplement has influence on gut. This meta-analysis includes 1221 experiments which has single enzyme studies and or studies with multiple enzyme complexes but not challenged. The ratio of Lactobacillus and E. coli is related to ADFI which showed comparatively lower negative correlation coefficient, with - 0.052 and - 0.035, respectively, whose I2 values are below 25%, showing that these studies show a significantly lower level of heterogeneity. Correlation between villus height, crypt depth, their ratio and fatty acid is also assessed, and it showed that when the animal is supplemented with two enzyme complexes resulted in positive gut health rather than the single or more than two enzymes.

A meta-analysis was performed on eight trials, which included a total of 992 parity 1 to 8 lactating sows, to evaluate the effects of feeding xylanase which is the main enzyme activity present in the enzymatic complex (Rovabio Excel, Adisseo, France) supplement throughout lactation on the following sow performance factors: BW loss, feed intake, backfat depth, and piglet growth. Even a short period of enzyme supplementation during lactation led to a reduction in BW loss of approximately 3 kg per sow (P = 0.003). This reduction represented 1-2% of the BW of sows. This effect could be explained by an increase in feed energy intake and enhanced feed digestibility. Sows fed enzyme-supplemented diets exhibited greater DM, OM, and GE digestibilities (3.4, 3.9, and 4.2% increases, respectively; P < 0.001) than sows fed control diets. During lactation, sows lost from 19 to 25 kg of BW (i.e., approximately 10% of their BW), with a difference between parity groups (P < 0.001). Body reserve mobilization was decreased in sows fed enzyme-supplemented diets (-2.9 kg, P = 0.003), with a more pronounced effect in primiparous than multiparous sows when BW loss is expressed relative to total BW (-2.27 vs. -0.59%, respectively; P = 0.058). Enzyme supplementation also increased litter weight gain up to weaning, with a greater effect in litters from multiparous sows than those from primiparous sows (5.4 vs. 0.6 kg, respectively; P = 0.009). These results could be explained in part by the relationship between their NE intake and either variations in BW or litter weight gain (R2 = 0.51 and 0.49, respectively; P < 0.001). Finally, the meta-analysis suggests that there are differences in the partitioning of the NE intake between growth and milk production and in relation to the sow's parity or physiological status. Extra energy released by enzyme is used for one of these functions (i.e., body mobilization reduction or greater milk export for litter gain).

OBJECTIVE: Our paper aimed to elucidate the mechanism of Weiwei granules in the treatment of Helicobacter pylori (Hp)-positive chronic atrophic gastritis (CAG) based on the TLR4/NF-κB/COX-2 inflammatory signaling pathway. METHODS: Hp-positive CAG patients were randomized into the control group (treated with quadruple therapy) or the observation group (treated with Weiwei granules based on the control group). The clinical efficacy, Hp clearance rate, and efficacy of traditional Chinese medicine (TCM) symptoms were compared between the two groups after six months of treatment. The scores of various histopathology variables, serum levels of inflammatory factors (interleukin-6 [IL-6], interleukin-8 [IL-8], and tumor necrosis factor-alpha [TNF-α]), gastrin-17 (G-17) and motilin (MTL), pepsinogen (PG) I and PG II, as well as serum levels of gastrointestinal hormone endothelin (ET), epidermal growth factor (EGF), and calcitonin gene-related peptide (CGRP), were compared between the two groups before and after treatment. TLR4, NF-κB, and COX-2 mRNA levels were compared in gastric mucosal tissues before and after treatment in the two groups. RESULTS: After treatment, the clinical efficacy, Hp clearance rate, and efficacy of TCM symptoms of patients in the observation group were higher than those in the control group. After treatment, the scores of various histopathology variables, serum levels of inflammatory factors (IL-6, IL-8, and TNF-α), gastrointestinal hormones (ET and EGF), and the expression levels of TLR4, NF-κB, and COX-2 mRNA in the gastric mucosal tissues were lower and G-17, MTL, CGRP, and PG I levels were higher in the observation group than in the control group. CONCLUSION: Weiwei granules can effectively improve Hp-positive CAG patients and reduce the expression levels of TLR4, NF-κB, and COX-2. ©The Author(s) 2024. Open Access. The enzyme transglutaminase 2 (TG2) plays a key role in celiac disease (CeD) pathogenesis. Active TG2 is located mainly extracellularly in the lamina propria but also in the villous enterocytes of the duodenum. The TG2 inhibitor ZED1227 is a promising drug candidate for treating CeD and is designed to block the TG2-catalyzed deamidation and crosslinking of gliadin peptides. Our aim was to study the accumulation of ZED1227 after oral administration of the drug. We studied duodenal biopsies derived from a phase 2a clinical drug trial using an antibody that detects ZED1227 when bound to the catalytic center of TG2. Human epithelial organoids were studied in vitro for the effect of ZED1227 on the activity of TG2 using the 5-biotin-pentylamine assay. The ZED1227-TG2 complex was found mainly in the villous enterocytes in post-treatment biopsies. The signal of ZED1227-TG2 was strongest in the luminal epithelial brush border, while the intensity of the signal in the lamina propria was only ~20% of that in the villous enterocytes. No signal specific to ZED1227 could be detected in pretreatment biopsies or in biopsies from patients randomized to the placebo treatment arm. ZED1227-TG2 staining co-localized with total TG2 and native and deamidated gliadin peptides on the enterocyte luminal surface. Inhibition of TG2 activity by ZED1227 was demonstrated in epithelial organoids. Our findings suggest that active TG2 is present at the luminal side of the villous epithelium and that inhibition of TG2 activity by ZED1227 occurs already there before gliadin peptides enter the lamina propria.

BACKGROUND: It was suggested that low salivary-amylase activity (SAA) and cooling or stir-frying cooked starch decreases its digestibility and glycemic index. OBJECTIVE: We determined the effects of SAA, cooling, and single-nucleotide polymorphisms (SNPs) in the salivary amylase (AMY1), pancreatic amylase (AMY2A, AMY2B), maltase-glucoamylase (MGAM), and sucrase-isomaltase (SI) genes on starch digestibility and glycemic index of cooked polished rice. METHODS: Healthy subjects [pilot, n = 12; main, n = 20 with low-SAA (<50 U/mL), and n = 20 with high-SAA (>105 U/mL)] consumed test meals containing 25 g (pilot) or 50 g (main) available carbohydrate at a contract research organization using open-label (pilot) or assessor-blinded (main), randomized, crossover, Latin-square designs (trial registration: NCT03667963). Pilot-trial test meals were dextrose, freshly cooked polished rice, cooked rice cooled overnight, stir-fried hot rice, or stir-fried cold rice. Main-trial test meals were dextrose, dextrose plus 10 g lactulose, plain hot rice, or plain cold rice. In both trials, blood glucose was measured fasting and at intervals over 2 h. In the main trial, breath hydrogen was measured fasting and hourly for 6 h to estimate in vivo starch digestibility. Data were analyzed by repeated-measures ANOVA for the main effects of temperature and stir-frying (pilot trial) or the main effects of SAA and temperature (main trial) and their interactions. Effects of 24 single nucleotide polymorphisms (SNPs) were assessed separately. Means were considered to be equivalent if the 95% CI of the differences were within ±20% of the comparator mean for glucose response/glycemic index or ±7% for digestibility. RESULTS: Pilot: neither temperature nor stir-frying significantly affected glucose incremental AUC (primary endpoint, n = 12). Main: mean ± SEM glycemic index (primary endpoint, n = 40) was equivalent for low-SAA compared with high-SAA (73 ± 3 vs. 75 ± 4) and cold rice compared with hot rice (75 ± 3 vs. 70 ± 3). Estimated starch digestibility (n = 39) was equivalent for low-SAA compared with high-SAA (95% ± 1% vs. 92% ± 1%) and hot rice compared with cold rice (94% ± 1% vs. 93% ± 1%). N

Starch digestion induces postprandial hyperglycemia, which can be modulated by polyphenols and polysaccharides via α-amylase inhibition. Current research focuses on individual α-amylase inhibitors, neglecting the activity changes of combined compounds induced by structural differences. This study explored the inhibitory effect and mechanism of the epigallocatechin gallate (EGCG) and exopolysaccharides from Bacillus subtilis STB22 (BS-EPS) complex on porcine pancreatic α-amylase (PPA). The results showed that EGCG (30 mg/mL) and BS-EPS (0.1 mg/mL) individually inhibited PPA by 44.47 % and 21.55 %, respectively, while their complex exhibited enhanced inhibition (72.95 %). The combination index ≤1 confirmed a synergistic and additive inhibitory effect. Mechanistic studies revealed that EGCG and BS-EPS acted as non-competitive and mixed-type inhibitors of PPA, respectively. When combined, they formed a monolayer adsorption complex that exhibited mixed-type inhibition on PPA and statically quenched PPA fluorescence via ternary complex formation. These findings provide a theoretical foundation for developing novel α-amylase inhibitors.

Drug-drug interactions (DDIs) pose significant challenges in pharmacotherapy, affecting drug efficacy and safety. Traditional in vitro and in vivo models often fail to accurately predict clinically relevant DDIs, necessitating the development of advanced testing platforms. This review explores cutting-edge in vitro and in vivo systems, including chimeric mice with humanized livers, clustered regularly interspaced short palindromic repeats-CRISPR-associated animal models, liver microphysiological systems, and 3-dimensional spheroids and organoids that enhance the assessment of DDIs. These models enable the precise evaluation of drug metabolism, enzyme induction/inhibition, and transporter-mediated interactions under physiologically relevant conditions. In addition, we discuss the latest advancements in predictive modeling techniques for DDIs, focusing on physiologically based pharmacokinetic models and machine learning approaches. Physiologically based pharmacokinetic models integrate drug-specific and system-specific parameters to simulate DDIs dynamically, bridging the gap between preclinical and clinical findings. Machine learning-based predictive tools use vast datasets to identify complex interaction patterns, improving DDI risk assessment in early drug development. By integrating these novel experimental and computational approaches, researchers can achieve more accurate, quantitative DDI predictions, facilitating safer drug design and regulatory decision-making. The review highlights these emerging methodologies, emphasizing the need for continued refinement to enhance their predictive power and translational relevance. Future research should focus on optimizing hybrid strategies that combine mechanistic and data-driven models to achieve robust, clinically meaningful DDI assessments. SIGNIFICANCE STATEMENT: This review showcases advanced experimental and computational tools to improve drug-drug interaction prediction. These innovations enhance drug-drug interaction accuracy, support safer drug development, aid regulatory and clinical decisions, reduce adverse reactions, and optimize patient care.

Alzheimer's disease (AD) is a complex neurodegenerative disorder that poses significant therapeutic challenges. Currently available treatments offer symptomatic relief but often yield suboptimal outcomes due to inter-individual variability in drug metabolism. Cytochrome P450 (CYP450) enzymes, particularly those exhibiting genetic polymorphisms, play a central role in the hepatic metabolism of many AD medications. This review focuses on the influence of CYP450 polymorphisms-specifically in CYP1A2, CYP2C9, CYP2C19, CYP2D6, and CYP3A4-on the pharmacokinetics, efficacy, and safety of approved anti-AD drugs. We discuss how variability in CYP450 expression and activity affects drug response, and examine the implications for adverse drug reactions, therapeutic failure, and dosage optimization. In addition, we evaluate current evidence for CYP450-mediated interactions with traditional Chinese medicines, which are increasingly used in complementary AD therapy. The potential for CYP450 genotyping and phenotyping to guide personalized treatment strategies is critically assessed. We argue that integrating pharmacogenomics into clinical practice may enhance therapeutic precision, reduce adverse outcomes, and improve quality of life in patients with AD. This review provides updated insight into the clinical significance of CYP450 polymorphisms in AD therapy and outlines future directions for personalized medicine approaches.

Erratum in AAPS J. 2025 May 16;27(4):92. doi: 10.1208/s12248-025-01081-2. During drug development, the design, interpretation and risk assessment of drug-drug interaction (DDI) are generally performed with physiologically-based pharmacokinetic (PBPK) modelling. Critical parameters are the hepatic metabolic fraction (fm) and intestinal availability (FG) which are commonly informed by clinical data. In this study, two methods for the simultaneous estimation of these parameters are proposed which utilize the distinctive changes in substrate's plasma concentration profiles in response to inhibition of intestinal and hepatic enzymes. The two-dimensional DDI (2D-DDI) method estimates the fm and FG values directly from the ratios of area-under-curve (AUCR) and maximum concentration (CmaxR), while the population PBPK method utilizes the full concentration-time data of a substrate without or with an inhibitor. The utility of both methods was demonstrated for a broad range of > 50,000 virtual and six actual CYP3A substrates. The 2D-DDI method is fast, reliable, and does not require a priori PBPK model development. The population PBPK method can estimate the population parameters and inter-individual variabilities of fm and FG and is applicable to more complex DDIs (e.g., multiple pathways/dynamic inhibitor concentration-time profiles) without the need for IV data. Like other approaches, both methods show an increasing uncertainty for substrates with high hepatic extraction and sensitivity to the assumed degree of enzyme inhibition. While both methods were evaluated for CYP3A substrates, the methodology equally applies to other enzymes. Additionally, this study provides guidance for clinical DDI study design to facilitate robust DDI extrapolation necessary to inform drug labels on concomitant medications in lieu of clinical trials. © 2025.
