The anti-inflammatory properties of the topical herbal composition VEL-091604 with gentian root, licorice root, and willow bark extract were assessed in a randomized, prospective, placebo-controlled, double-blind ultraviolet (UV)-erythema test study with 42 healthy volunteers in comparison to 1% hydrocortisone acetate. The efficacy and tolerability of VEL-091604 cream 2 times daily over 2 wk was evaluated in an open-label, prospective proof of concept study in 10 subjects with atopic dermatitis using a lesional SCORAD severity score. In the UV-erythema test VEL-091604 cream significantly reduced inflammation compared to placebo and was as effective as 1% hydrocortisone acetate. The clinical study with atopic subjects revealed a significant and rapid reduction of the lesional SCORAD severity score in the test areas after 1 and 2 wk. No adverse events were recorded. It is concluded that the herbal cream VEL-091604 with licorice root, willow bark, and gentian root extract display anti-inflammatory properties in vivo. It is a promising new treatment option for atopic dermatitis that warrants further investigation in controlled studies. Georg Thieme Verlag KG Stuttgart · New York.

Gentiana scabra Bge., Gentiana triflora Pall., Gentiana manshurica Kitag., and Gentiana rigescens Franch. were recorded as Gentianae Radix et Rhizome (Longdan) in Chinese Pharmacopoeia (ChP) and controlled by gentiopicroside (GPS). Owing to similar contents of GPS among these herbs, the demand for an effective method for Longdan classification and quality control remains urgent. In this study, we prepared the extract rich in phenolic components from four species herbs. UPLC-Q-TOF-MS analysis and bioactive evaluation revealed that Longdan phenolic extracts possess distinct phytochemical profiles and anti-inflammatory efficacy. After that, mangiferin, kaempferol, ferulic acid, and esculetin were selected as bioactivity-related components and potential quality markers for Longdan herbs. Subsequently, 16 batches of Longdan materials were quantitatively analyzed based on the contents of characteristic phenolic compounds. By means of chemometric analysis (circular heat map, principal component analysis [PCA], and hierarchical clustering), 16 batches of Longdan herbs could be well classified according to differences in phenolic index compounds (PICs). Furthermore, with the help of network pharmacology analysis, the underlying in vivo targets and gene pathways of PICs were illustrated. Thus, the systematic importance of Longdan phenolic components and usefulness of PICs for herbs differentiating were clarified and the established method is helpful for the quality control of Gentianae Radix et Rhizome. © 2026 Wiley‐VHCA AG.

INTRODUCTION: Cancer patients commonly suffer from substantial side effects of oncological therapies. Therefore, the Oncology Working Group of the Competence Network for Integrative Medicine in Baden-Württemberg, Germany (KIM-BW) developed practice-oriented recommendations for the integrative treatment of chemotherapy-induced mucositis (CIM), nausea and vomiting (CINV), and cancer-related fatigue (CRF). METHODS: Two expert groups of physicians and nurses developed therapeutic recommendations using an interdisciplinary expert consensus process oriented on a Delphi-methodology with a standardized scoring matrix, considering training, feasibility, time intensity, clinical effectiveness, contraindications, and interactions. The consensus process was complemented by a targeted, non-systematic literature search conducted across the AWMF S3 Guideline on Complementary Medicine in Oncology, the KOKON knowledge database, the Working Group on Integrative Care in Oncology, and PubMed/Medline. RESULTS: The expert panel consisted of 21 professionals (14 physicians, 7 nurses), all conventionally trained with additional qualifications in integrative disciplines. We evaluated 83 interventions. Top recommendations were identified for each symptom. For CIM: sage tea mouth rinses, ice cubes, sea buckthorn oil mouth rinses, frozen pineapple cubes, and herbal oral balm. For CRF: movement therapy, yarrow liver compresses, viscum album therapy, sleep hygiene with regular circadian rhythms, and hydrotherapy. For CINV: acupressure, ginger, aromatherapy, bitter botanicals such as gentian root, and homeopathic preparation nux vomica. CONCLUSIONS: Integrative treatment recommendations developed by the KIM Oncology Working Group provide pragmatic, clinically grounded guidance for integrative management of common treatment-related symptoms in oncology. Prospective evaluation of safety, effectiveness, and implementation across settings is warranted.

Persistent singultus (hiccups) can be distressing and challenging to manage in the postoperative setting. While pharmacological interventions such as metoclopramide are commonly used, their effectiveness is often limited. Gentian root (Gentiana lutea) contains intensely bitter secoiridoid compounds known to stimulate vagally mediated reflexes and has a long history of use for digestive complaints. We report a case of a 70-year-old male who developed persistent hiccups following minor urologic surgery under propofol anesthesia. The hiccups were unresponsive to metoclopramide. The patient then self-administered a small amount (~20 mL) of traditional gentian schnapps, resulting in complete cessation of hiccups within 1 min. The effect was reproducible upon two subsequent recurrences later the same day. No further episodes occurred after the third administration. This case raises the hypothesis that bitter taste receptor activation in the oropharyngeal and gastrointestinal tract may modulate the hiccup reflex arc. While spontaneous resolution or the effect of alcohol cannot be excluded, the rapid and reproducible response suggests a possible therapeutic role for bitter-tasting botanical preparations in functional singultus. © 2025. Gentiana pupurea L. (Gentianaceae) is an important medicinal plant in Norway and the bitter roots were used against a number of diseases, often related to gastrointestinal ailments and respiratory diseases. Gentian species are reported to be used against malaria in several geographic regions, and in vitro and computational experiments indicate that secoiridoids have an anti-plasmodial effect. Three previously undescribed compounds; the biphenyl secoiridoid glycoside 5''-deoxyamaroswerin, the benzophenone 6''-O-sinapoyl-2,4,3'-trihydroxy-benzophenone-6-O-β-glucopyranoside, and the benzyl benzoate (3-hydroxy-2-methoxybenzyl)-2,3-dihydroxybenzoate, together with eleven other low-molecular compounds were isolated from the 70 % ethanol extract of G. purpurea roots. The structures were elucidated by 1D and 2D NMR methods and high-resolution mass spectrometry. No in vitro anti-plasmodial activity was observed for the 70 % ethanol extract, swertiamarin, sweroside, gentiopicrin, amaroswerin, amarogentin, 5''-deoxyamaroswerin, and amaropanin obtained from G. purpurea.

Background/Objectives: This study aimed to develop gastroretentive tablets based on mucoadhesive-floating systems with encapsulated gentian (Gentiana lutea, Gentianaceae) root extract to overcome the low bioavailability and short elimination half-life of gentiopicroside, a dominant bioactive compound with systemic effect. The formulation also aimed to promote the local action of the extract in the stomach. Methods: Tablets were obtained by direct compression of sodium bicarbonate (7.5%) and solid lipid microparticles (92.5%), which were obtained with lyophilizing double emulsions. A quality by design (QbD) was employed to evaluate the impact of formulation factors and processing parameters on emulsion viscosity, powder characteristics (moisture content, encapsulation efficiency, flowability), and tablet characteristics (floating lag time, gentiopicroside release, and assessment of dispersibility during in vitro dissolution). Results: The trehalose content and high-shear-homogenization (HSH) time of primary emulsion were critical factors. Trehalose content positively influenced emulsion viscosity, moisture content, floating lag time, encapsulation efficiency, and the release rate of gentiopicroside. HSH time positively affected powder stability and negatively gentiopicroside release. The selected powder had a high gentiopicroside encapsulation efficiency (95.13%), optimal stability, and good flowability. The developed tablets exhibited adequate floating lag time (275 s), mucoadhesive properties, and gentiopicroside biphasic release (29.04% in 45 min; 67.95% in 6 h). Furthermore, the optimal tablet formulation remained stable for 18 months and was primarily digested by duodenal enzymes. Conclusions: Dual-mechanism gastroretentive tablets with encapsulated gentian root extract were successfully developed. The in vitro digestion study demonstrated that the optimal formulation effectively resisted gastric enzymes, ensuring the release of its contents in the small intestine, even in the case of premature gastric evacuation.

A soil removal device for the root-soil complex of Gentian imitating the percussion function of a woodpecker was designed to improve the soil removal efficiency of harvesting devices for rhizome-type traditional Chinese herbal medicines. Based on the physical parameters of roots and the root-soil complex of Gentian, the structure parameters of the striking arm and the actual profile of the cam are determined according to the physical parameters when the woodpecker knocks on the tree. The key parameters that affect the working performance of the soil removal device and their suitable value ranges have been identified through the impact test and analysis of the root-soil complex of Gentian. The mass of the striking hammer, the swing angle of the striking arm, and the rotation speed of the cam were taken as the experimental factors and the soil removal rate and the energy consumption per hammer percussion were taken as the experimental indicators. The ternary quadratic orthogonal regression combination experiment was carried out using Design-Expert. The regression model of the influence factors and evaluation indicators was established through the analysis of variance. The interaction effects of the influence factors on the indicators were analyzed using the response surface method. Using multiobjective optimization method, the optimal parameter combination was obtained as that of the mass of the striking hammer of 0.9 kg, the swing angle of the striking arm of 47°, and the rotation speed of the cam of 100 r/min, then the soil removal rate was the maximum and the energy consumption of single-hammer knocking was the minimum, with the values of 89.12% and 31.21 J, respectively. This study can provide a reference for the design and optimization of soil removal devices for rhizome-type traditional Chinese herbal medicines.

ETHNOPHARMACOLOGICAL RELEVANCE: Gentiana radix (GR) and wine-processed Gentiana radix (WGR) have been commonly used in folk medicine for the treatment of bile or liver disorders, including jaundice, hepatitis, swelling and inflammation for thousands of years. However, the therapeutic effects of gentian root (GR) and wine-made gentian root (WGR) treatment on damp-heat jaundice syndrome (DHJS) have not been studied in animal experiments. AIM OF THE STUDY: This study aimed to investigate the protective effects and mechanisms of GR and WGR on DHJS in rats. MATERIALS AND METHODS: In a high-fat and high-sugar diet in a humidified hot environment, hepatic injury induced by giving alpha-naphthalene isothiocyanate (ANIT) in rats were used as a DHJS model. Histological analysis, enzyme-linked immunosorbent assay (ELISA), PCR analysis, and metabolomics were used to elucidate the mechanism of GR and WGR for DHJS. RESULTS: The results indicated that GR and WGR affected DHJS by inhibiting the release of aspartate aminotransferase (AST), alanine aminotransferase (ALT), alkaline phosphatase (ALP), direct bilirubin (D-BIL), total bilirubin (TBIL), total bile acid (TBA), malondialdehyde (MDA), glutathione S-transferase (GST) (P < 0.05). In addition, they significantly reduced the gene expression levels of Na+/taurocholate cotransporting polypeptide (NTCP), bile salt export pump (BESP), multidrug resistance-associated protein 2 (MRP2) and multidrug resistance-associated protein 3 (MRP3) (P < 0.05). The WGR group improved the above function indicators better than the GR group. GR and WGR could restore 11 potential biomarkers in rats with DHJS tended to return to normal levels, these biomarkers were involved in arachidonic acid metabolism, steroid hormone biosynthesis, biosynthesis of unsaturated fatty acids, porphyrin and chlorophyll metabolism, retinol metabolism, arginine biosynthesis. The results of the metabolic pathway showed that WGR was significantly better than GR in the improvement of porphyrin and chlorophyll metabolism. CONCLUSIONS: These findings suggest that treatment with GR and WGR has a beneficial effect on DHJS in rats, the major mechanisms may be involved in impro

The dried roots and rhizomes of Radix Gentianae Macrophyllae are widely used as food material or medicinal crops. "Sweating" is a traditional postharvest processing method, the basic processing procedure consists of softening, stacking and drying. The aim of this paper is to unveil the scientific connotation responsible for the "Sweating" processing in Radix Gentianae Macrophyllae during postharvest. Thus, the effect of different postharvest processing methods on the metabolic pathways of Radix Gentiasnae Macrophyllae was studied by the non-targeted metabolomic technique in combination with the label-free proteomics approach. The results showed that the differentially accumulated metabolites and abundant proteins were mainly enriched in the pathways of phenylalanine, tyrosine and tryptophan biosynthesis, polyphenols and terpenoids biosynthesis. "Sweating" has a greater up-regulation effect on these pathways than "Non-sweating", and can induce protein expression and metabolite accumulation associated with the quality traits of Radix Gentianae Macrophyllae. The results provide a detailed explanation of the scientific connotation of crucial steps of "Sweating" processing wherein opportunities existed for taking appropriate measures to enhance the accumulation of bioactive ingredients. These findings will serve as significant references for enhancing the postharvest processing technology of Radix Gentianae Macrophyllae and similar plants, resulting in higher product quality for food or plant materials production.

(1) Background: Establishment of a method for evaluating Gentianae Radix et Rhizoma (GRR) classes based on chemical composition and core efficacy; (2) Methods: Liquid chromatography-mass spectrometry (LC-MS) was used to determine the chemical constituents of GRR-first class (GF) and GRR-second class (GS). The cell viability, liver function, oxidative stress enzyme activity, and inflammatory factor levels of GF and GS on H2O2-induced HepG2 cells were determined with CCK-8, ELISA, and biochemical methods, and the antioxidant activity of the two was evaluated using bioefficacy; ELISA, biochemical methods, real-time fluorescence quantitative polymerase chain reaction (RT-qPCR) method, and Western blot (WB) were used to determine the liver function, oxidative stress enzyme activity, inflammatory factor levels, and expression of related genes and proteins in mice with acute liver injury (ALI) model induced with 0.3% CCl4 olive oil solution after gavage administration; (3) Results: GF and GS had the same types of components, but the cyclic enol ether terpenes such as morinlon goside c, loganin, gentiopicroside, and swertiamarin differed significantly between the two; the effect of GF on CCl4-induced acute hepatic injury in C57BL/6 mice was stronger compared to GS. It helped alleviate weight loss, increase hepatic and splenic indices, improve hepatic lobular structure and hepatocyte status, inhibit collagen deposition, enhance oxidative stress and anti-inflammatory-related genes and protein expression, and decrease apoptotic genes and proteins more significantly than GS; (4) Conclusions: In this study, we established a GRR class evaluation method combining chemical composition and core medicinal effects, which can rapidly determine the differential composition of GF and GS, detect the quality of GRR thr
