OBJECTIVE: This study aims to systematically evaluate the synergistic effects of long-term supplementation of L-citrulline combined with exercise on body composition in adults through a meta-analysis. METHODS: This study primarily summarized and analyzed outcome indicators including body weight (BW), body mass index (BMI), body fat (BF) (including fat mass [FM] and body fat percentage [BFP]), android fat mass (AFM), gynoid fat mass (GFM), and total lean mass (TLM). Statistical analyses were performed using RevMan (Version 5.4) software provided by the Cochrane Collaboration. For continuous variables, the pooled effect sizes were expressed as standardized mean differences (SMD) with their 95% confidence intervals (CI). RESULTS: A total of 9 randomized controlled trials (RCTs) were included that investigated the effects of long-term supplementation of L-citrulline combined with exercise on body composition in adults. The results indicated that, compared to exercise alone, L-citrulline supplementation combined with exercise significantly improved BF (SMD = -0.21, 95% CI: -0.40 to -0.01, P = 0.03) and AFM (SMD = -0.32, 95% CI: -0.57 to -0.07, P = 0.01). However, no significant effects were observed on BW (SMD = -0.01, 95% CI: -0.27-0.25, P = 0.95), BMI (SMD = 0.01, 95% CI: -0.31-0.33, P = 0.93), GFM (SMD = -0.17, 95% CI: -0.42-0.08, P = 0.18), and TLM (SMD = -0.02, 95% CI: -0.23-0.19, P = 0.83). CONCLUSION: This study is the first to demonstrate through a meta-analysis that long-term supplementation of L-citrulline combined with exercise can synergistically improve BF and AFM in adults.

BACKGROUND/OBJECTIVES: L-Citrulline (CIT) is a non-essential amino acid abundant in watermelon and commonly used as a dietary supplement to enhance exercise performance. Although its benefits for endurance and resistance training are well documented, its effects on body composition remain uncertain. This systematic review and dose-response meta-analysis aimed to assess the impact of CIT supplementation on anthropometric parameters. METHODS: A comprehensive search of major databases identified relevant randomized controlled trials (RCTs) published until March 2025. A random-effects model was used to synthesize the data. RESULTS: Twenty-one RCTs were included. Overall, CIT supplementation had no substantial effects on body mass index (BMI), body weight, fat mass (FM), waist circumference (WC), body fat percentage (BFP), and fat-free mass (FFM). Subgroup analyses revealed reductions in FM among participants over 40 years of age and in those administered more than 6 g/day of CIT. Interventions lasting 3 to 8 weeks were associated with a significant increase in FFM. Dose-response analyses suggested a non-linear association between CIT supplementation duration and changes in FM and FFM. CONCLUSIONS: CIT supplementation appears to have no overall effect on body composition. However, exploratory findings indicated potential benefits at higher doses or shorter durations. Rigorous trials controlling for dietary intake and training variables are needed to clarify its long-term effects.

OBJECTIVE: This study aims to investigate the effects of l-citrulline supplementation and watermelon intake on blood pressure (BP) in middle-aged and elderly individuals. METHODS: Data analysis was conducted using RevMan 5.4.1 software, employing a random effects model to statistically analyze the pooled effect sizes of the obtained systolic blood pressure (SBP) and diastolic blood pressure (DBP) data. Results are reported as weighted mean differences (WMD) with 95 % confidence intervals (95 % CI). RESULTS: A total of 15 randomized controlled trials (RCTs) investigating the effects of l-citrulline supplementation and watermelon intake on BP in middle-aged and elderly individuals were included, comprising 24 datasets involving 415 participants. The results indicate that l-citrulline supplementation and watermelon intake significantly reduced SBP (-4.02 mmHg [95 % CI: -6.54 to -1.50], P = 0.002) and DBP (-2.54 mmHg [95 % CI: -4.27 to -0.81], P = 0.004) in the elderly. Subgroup analysis showed that the combined supplementation of l-citrulline and l-arginine significantly decreased SBP (-10.44 mmHg [95 % CI: -13.57 to -7.31], P < 0.00001) and DBP (-4.86 mmHg [95 % CI: -7.93 to -1.79], P = 0.002) in this population. CONCLUSION: l-citrulline supplementation and watermelon intake overall lowered BP in middle-aged and elderly individuals. Subgroup analysis indicates that the combined supplementation of l-citrulline and l-arginine is superior to l-citrulline or watermelon supplementation alone. These findings provide new dietary recommendations for BP management in the elderly, suggesting that dietary interventions can effectively improve cardiovascular health and reduce the risk of cardiovascular disease (CVD).

BACKGROUND: Evidence suggests L-arginine may be effective at reducing pre-eclampsia and related outcomes. However, whether L-arginine can prevent or only treat pre-eclampsia, and thus the target population and timing of initiation, remains unknown. OBJECTIVES: To evaluate the effects of L-arginine and L-citrulline (precursor of L-arginine) on the prevention and treatment of pre-eclampsia. SEARCH STRATEGY: MEDLINE, Embase, CINAHL, Global Index Medicus and the Cochrane Library were searched through 7 February 2024. SELECTION CRITERIA: Trials administering L-arginine or L-citrulline to pregnant women, with the comparison group receiving placebo or standard care, were included. DATA COLLECTION AND ANALYSIS: Meta-analyses were conducted separately for prevention or treatment trials, using random-effects models. MAIN RESULTS: Twenty randomised controlled trials (RCTs) (2028 women) and three non-randomised trials (189 women) were included. The risk of bias was 'high' in eight RCTs and showed 'some concerns' in 12. In prevention trials, L-arginine was associated with a reduced risk of pre-eclampsia (relative risk [RR] 0.52; 95% confidence interval [CI], 0.35, 0.78; low-certainty evidence, four trials) and severe pre-eclampsia (RR 0.23; 95% CI, 0.09, 0.55; low-certainty evidence, three trials). In treatment trials, L-arginine may reduce mean systolic blood pressure (MD -5.64 mmHg; 95% CI, -10.66, -0.62; very low-certainty evidence, three trials) and fetal growth restriction (RR 0.46; 95% CI, 0.26, 0.81; low-certainty evidence, two trials). Only one study (36 women) examined L-citrulline and reported no effect on pre-eclampsia or blood pressure. CONCLUSIONS: L-arginine may be promising for pre-eclampsia prevention and treatment, but findings should be interpreted cautiously. More trials are needed to determine the optimal dose and time to commence supplementation and support clinical decision-making. © 2025 The Author(s). The repercussions on oxidative and inflammatory stress markers under the effects of arginine and citrulline in response to exercise are not fully reached. We completed a systematic review to investigate the effects of L-Citrulline or L-Arginine on oxidative stress and inflammatory biomarkers following exercise. EMBASE, MEDLINE (PubMed), Cochrane Library, CINAHL, LILACS, and Web of Science databases were used to record the trials. This study includes randomized controlled trials (RCTs) and non-RCTs with subjects over 18 years old. Those under the intervention protocol consumed L-Citrulline or L-Arginine, and the controls ingested placebo. We recognized 1080 studies, but only 7 were included (7 studies in meta-analysis). We observed no difference between pre- vs. post-exercise for oxidative stress (subtotal = -0.21 [CI: -0.56, 0.14], p = 0.24, and heterogeneity = 0%. In the sub-group "L-Arginine" we found a subtotal = -0.29 [-0.71, 0.12], p = 0.16, and heterogeneity = 0%. For the "L-Citrulline" subgroup we observed a subtotal = 0.00 [-0.67, 0.67], p = 1.00, and heterogeneity was not applicable. No differences were observed between groups (p = 0.47), and I² = 0%) or in antioxidant activity (subtotal = -0.28 [-1.65, 1.08], p = 0.68, and heterogeneity = 0%). In the "L-Arginine" sub-group, we found a subtotal = -3.90 [-14.18, 6.38], p = 0.46, and heterogeneity was not applicable. For the "L-Citrulline" subgroup, we reported a subtotal = -0.22 [-1.60, 1.16], p = 0.75, and heterogeneity was not applicable. No differences were observed between groups (p = 0.49), and I² = 0%), inflammatory markers (subtotal = 8.38 [-0.02, 16.78], p = 0.05, and heterogeneity = 93%. Tests for subgroup differences were not applicable, and anti-inflammatory markers (subtotal = -0.38 [-1.15, 0.39], p = 0.34 and heterogeneity = 15%; testing for subgroup differences was not applicable). In conclusion, our systematic review and meta-analysis found that L-Citrulline and L-Arginine did not influence inflammatory biomarkers and oxidative stress after exercise.

BACKGROUND: Increasing nitric oxide bioavailability may induce physiological effects that enhance endurance exercise performance. This review sought to evaluate the performance effects of consuming foods containing compounds that may promote nitric oxide bioavailability. METHODS: Scopus, Web of Science, Ovid Medline, EMBASE and SportDiscus were searched, with included studies assessing endurance performance following consumption of foods containing nitrate, L-arginine, L-citrulline or polyphenols. Random effects meta-analysis was conducted, with subgroup analyses performed based on food sources, sex, fitness, performance test type and supplementation protocol (e.g. duration). RESULTS: One hundred and eighteen studies were included in the meta-analysis, which encompassed 59 polyphenol studies, 56 nitrate studies and three L-citrulline studies. No effect on exercise performance following consumption of foods rich in L-citrulline was identified (SMD=-0.03, p=0.24). Trivial but significant benefits were demonstrated for consumption of nitrate and polyphenol-rich foods (SMD=0.15 and 0.17, respectively, p<0.001), including performance in time-trial, time-to-exhaustion and intermittent-type tests, and following both acute and multiple-day supplementation, but no effect of nitrate or polyphenol consumption was found in females. Among nitrate-rich foods, beneficial effects were seen for beetroot, but not red spinach or Swiss chard and rhubarb. For polyphenol-rich foods, benefits were found for grape, (nitrate-depleted) beetroot, French maritime pine, Montmorency cherry and pomegranate, while no significant effects were evident for New Zealand blackcurrant, cocoa, ginseng, green tea or raisins. Considerable heterogeneity between polyphenol studies may reflect food-specific effects or differences in study designs and subject characteristics. Well-trained males (V̇O2max ≥65 ml.kg.min-1) exhibited small, significant benefits following polyphenol, but not nitrate consumption. CONCLUSION: Foods rich in polyphenols and nitrate provide trivial benefits for endurance exercise performance, although these effects may be food dependent. Highly trained endurance athletes do not appea

Citrulline malate (CitMal) is a dietary supplement that is suggested to enhance strength training performance. However, there is conflicting evidence on this matter. Thus, the purpose of this meta-analysis was to determine whether supplementing with CitMal prior to strength training could increase the total number of repetitions performed before reaching voluntary muscular failure. A systematic search was conducted wherein the inclusion criteria were double-blind, placebo-controlled studies in healthy participants that examined the effect of CitMal on repetitions to failure during upper body and lower body resistance exercises. The Hedges's g standardized mean differences (SMD) between the placebo and CitMal trials were calculated and used in a random effect model. Two separate subanalyses were performed for upper body and lower body exercises. Eight studies, including 137 participants who consisted of strength-trained men (n = 101) and women (n = 26) in addition to untrained men (n = 9), fulfilled the inclusion criteria. Across the studies, 14 single-joint and multijoint exercises were performed with an average of 51 ± 23 total repetitions during 5 ± 3 sets per exercise at ∼70% of one-repetition maximum. Supplementing with 6-8 g of CitMal 40-60 min before exercise increased repetitions by 3 ± 5 (6.4 ± 7.9%) compared with placebo (p = .022) with a small SMD (0.196). The subanalysis for the lower body resulted in a tendency for an effect of the supplement (8.1 ± 8.4%, SMD: 0.27, p = .051) with no significant effect for the upper body (5.7 ± 8.4%, SMD: 0.16, p = .131). The current analysis observed a small ergogenic effect of CitMal compared with placebo. Acute CitMal supplementation may, therefore, delay fatigue and enhance muscle endurance during high-intensity strength training.

BACKGROUND: Citrulline is one of the non-essential amino acids that is thought to improve exercise performance and reduce post-exercise muscle soreness. We conducted a systematic review and meta-analysis to determine the effect of citrulline supplements on the post-exercise rating of perceived exertion (RPE), muscle soreness, and blood lactate levels. METHODS: A random effects model was used to calculate the effect sizes due to the high variability in the study design and study populations of the articles included. A systematic search of PubMed, Web of Science, and ClinicalTrials.gov was performed. Eligibility for study inclusion was limited to studies that were randomized controlled trials involving healthy individuals and that investigated the acute effect of citrulline supplements on RPE, muscle soreness, and blood lactate levels. The supplementation time frame was limited to 2 h before exercise. The types and number of participants, types of exercise tests performed, supplementation protocols for L-citrulline or citrulline malate, and primary (RPE and muscle soreness) and secondary (blood lactate level) study outcomes were extracted from the identified studies. RESULTS: The analysis included 13 eligible articles including a total of 206 participants. The most frequent dosage used in the studies was 8 g of citrulline malate. Citrulline supplementation significantly reduced RPE (n = 7, p = 0.03) and muscle soreness 24-h and 48-h after post-exercise (n = 7, p = 0.04; n = 6, p = 0.25, respectively). However, citrulline supplementation did not significantly reduce muscle soreness 72-h post-exercise (n = 4, p = 0.62) or lower blood lactate levels (n = 8, p = 0.17). CONCLUSION: Citrulline supplements significantly reduced post-exercise RPE and muscle soreness without affecting blood lactate levels.

PURPOSE OF REVIEW: Several interventional studies assessed the impact of dietary supplements on blood pressure. The aim of the present systematic review and meta-analysis was to determine the efficacy of L-citrulline supplementation on blood pressure. RECENT FINDING: Pubmed, Scopus, Google Scholar, and ISI Web of Science were comprehensively searched until May 2018 to assess whether L-citrulline reduces blood pressure. Human clinical trials which reported the effect of L-citrulline supplementation on aortic and brachial blood pressure were included. Characteristics of studies and potential sources of heterogeneity were tabulated. A subgroup analysis was performed to attenuate observed inter-study heterogeneity. A total of five interventions were found for meta-analysis. The impact of L-citrulline on brachial systolic (change 0.28 mmHg; 95% CI - 2.87, 2.31 mmHg) and diastolic (change - 1.56 mmHg; 95% CI - 4.32, 1.20 mmHg) blood pressure was not significant. Also, there was no changes in aortic systolic (change 0.22 mmHg; 95% CI - 4.81, 4.38 mmHg) and diastolic (change 0.26 mmHg; 95% CI - 2.27, 2.80 mmHg) blood pressure after L-citrulline supplementation. Participants' body weight status was a source of observed heterogeneity. The present systematic review and meta-analysis suggests that L-citrulline supplementation had no beneficial effect on blood pressure.

BACKGROUND: High-intensity interval training (HIIT) with or without L-citrulline (CIT) oral supplementation improves body composition, functional capacities and muscle health in obese older adults, potentially through microRNA-driven regulation. We aimed to (1) investigate the impact of a 12-week HIIT on the expression level of microRNAs in vastus lateralis muscle biopsies and serum of obese older adults and (2) assess whether the differential expression level of microRNAs was associated with clinico-biological adaptations to HIIT and provide potential biomarkers of HIIT response. METHODS: In this secondary exploratory analysis of a double-blind randomized trial, 36 women and 32 men (67.2 ± 5.2 years) following 12 weeks HIIT randomized in two groups were supplemented daily with CIT (HIIT-CIT, n = 37) or with placebo (HIIT-PLA, n = 31). Phenotypic variables, serum parameters, muscle biopsies and subcutaneous abdominal adipose tissue outcomes were collected pre-intervention and postintervention. To assess the microRNA profile, the miRNome of muscle biopsy and serum was analysed using next generation sequencing in participants' subsets (n = 13). The microRNAs' differential expression level was analysed pre-intervention and postintervention by TaqMan-real-time qPCR in 68 participants. RESULTS: The expression of myo-microRNAs (miR-133a, b, -1, -206) and muscle-related-microRNAs (miR-499, -208) was not altered following HIIT with or without CIT. In HIIT-PLA, HIIT-CIT and subgroups (based on sex, age, body mass index, dynapenic status), the change in muscle (miR-504-5p, -744-5p, -151a-3p, -106b-5p, -127-5p) and circulating (miR-4433b-5p, 151a-3p, -744-5p, 483-3p, -106b-5p, -484) microRNA levels was associated with changes of clinico-biological parameters. Supplementing HIIT with CIT decreased the muscle miR-504-5p level (p = 0.022), correlating with lower body fat, improved functional capacities, muscle power and increased IGF-1 level (r = -0.6, p < 0.05). MiR-744-5p expression increased in dynapenic participants (p = 0.04), associated with lean mass gain (r = 0.50, p < 0.05), while miR-151a-3p downregulation in men's muscle (p = 0.01) was associated with better insuli
