BACKGROUND: Over-the-counter supplements are commonly used to manage chronic constipation; however, their efficacy remains unclear. We aimed to investigate the effect of food, vitamin or mineral supplements on stool output, gut transit time, symptoms, and quality of life in adults with chronic constipation via a systematic review and meta-analysis of randomized controlled trials (RCTs). METHODS: Studies were identified using electronic databases, backward citation, and hand-searching abstracts. RCTs reporting administration of food supplements (e.g., fruit extract supplements), vitamin or mineral supplements in adults with chronic constipation were included. Studies administering whole foods (e.g., fruits) were excluded. Risk of bias (RoB) was assessed with Cochrane RoB 2.0. Relative risks (RR), mean differences (MD), or standardized mean differences (95% confidence intervals [CI]) were calculated using a random-effects model. KEY RESULTS: Eight RCTs (787 participants) were included, investigating kiwifruit (n = 3 RCTs), senna (n = 2), magnesium oxide (n = 2), Ziziphus jujuba (n = 1), and Malva Sylvestris (n = 1) supplements. Kiwifruit supplements did not impact stool frequency (MD 0.24 bowel movements/week [-0.32, 0.80]; p = 0.40) or consistency (MD -0.11 Bristol points [-0.31, 0.09], p = 0.29). Overall, 61% responded to senna and 28% to control; however, this did not reach statistical significance (RR 2.78, [0.93, 8.27]; p = 0.07). Overall, 68% responded to magnesium oxide and 19% to control (RR 3.32 [1.59, 6.92]; p = 0.001). Magnesium oxide improved stool frequency (MD 3.72 bowel movements/week [1.41, 6.03]; p = 0.002) and consistency (MD 1.14 Bristol points [0.48, 1.79]; p = 0.0007). CONCLUSIONS AND INFERENCES: Magnesium oxide supplements are effective at improving cardinal symptoms of chronic constipation. Senna and kiwifruit supplements did not impact symptoms; however, findings were based on a small number of studies. Further research is required to investigate the effect of food supplements (e.g., kiwifruit supplements), as well as their whole food equivalents (e.g., whole kiwifruits) in chronic constipation. © 2023 The Authors. BACKGROUND: Baricitinib, a Janus kinase inhibitor with an established safety profile across multiple indications, has demonstrated efficacy in the treatment of severe alopecia areata with high maintenance of response during the initial treatment extension period. OBJECTIVES: To evaluate efficacy and safety of baricitinib for adults with severe alopecia areata through 152 weeks of continuous therapy. METHODS: The analysis includes patients from the BRAVE-AA1 and BRAVE-AA2 phase 3, randomized, double-blind, clinical trials: adults with Severity of Alopecia Tool (SALT) scores ≥50 who achieved SALT score ≤20 at week 52 (responders) and were continued on the same dose of baricitinib up to 152 weeks. RESULTS: Among week 52 responders who were continuously treated with baricitinib 4 mg or baricitinib 2 mg, 115 of 129 (89.1%) and 56 of 67 (83.6%), respectively, maintained SALT score ≤20 at week 152. The incidence rate of adverse events of special interest relative to earlier safety reports, remained consistent. LIMITATIONS: The analysis does not include outcomes of patients without SALT score ≤20 response at week 52, who continued therapy post-week 52. CONCLUSIONS: Over 152 weeks, patients stably maintained and extended their regrowth across scalp, eyebrows, and eyelashes with continued baricitinib treatment. There were no new safety signals.

BACKGROUND: Behavioral therapy using electromyography-based biofeedback (BIO) and botulinum toxin type A (BTA) injections are potential treatments for managing awake bruxism (AB) in patients with temporomandibular disorders (TMD). The aim of this study was to compare the effectiveness of BIO and BTA in reducing AB behaviors, pain intensity, and psychosocial distress in individuals with TMD. METHODS: This single-center, single-blind, parallel-group randomized clinical trial enrolled 40 adults with TMD diagnosed with AB. AB was assessed using the Oral Behavior Checklist (OBC) and smartphone-based Ecological Momentary Assessment (EMA). Participants presenting an AB frequency of ≥ 60% were randomly allocated to receive BIO or BTA intervention. Pain intensity was measured by the Characteristic Pain Intensity (CPI) score, and psychosocial status was evaluated using the Patient Health Questionnaires (PHQ-9, PHQ-15) and the Generalized Anxiety Disorder questionnaire (GAD-7). Assessments were conducted at baseline, and at 1-, 3-, and 6-month follow-ups. Statistical analyses included Mann-Whitney U for AB behaviors, Wilcoxon Signed Rank test for within-group comparisons, and the Kruskal-Wallis test followed by the Dwass-Steel-Critchlow-Fligner post hoc test for between-group comparisons. RESULTS: Intra-group comparisons showed a significant reduction in BIO group for sustained teeth contact (BIO: p = 0.004) and EMA-assessed AB global behavior (BIO: p = 0.008). No significant improvements were found in BTA group for the same variables in all follow-ups (p > 0.05). However, no significant differences were found between groups in all follow-ups for OBC and EMA scores (p > 0.05). Similarly, there were no significant differences between groups for CPI (p = 0.39) or psychosocial outcomes (p > 0.05). CONCLUSION: Even though neither intervention improved pain nor psychosocial outcomes, BIO significantly reduced AB behaviors, indicating that BIO may be preferable for controlling AB behaviors in TMD patients. BACKGROUND: Frontal fibrosing alopecia (FFA) is a cicatricial alopecia with generally poor prognosis if untreated and no approved treatment options. OBJECTIVE: The aim of this study was to evaluate changes in the molecular signature of FFA lesions after application of delgocitinib cream. Safety, tolerability, and efficacy were also investigated. METHODS: This was a phase 2a, randomized, double-blind, exploratory trial of 20 mg/g delgocitinib cream (2%) versus cream vehicle in patients with FFA. RESULTS: A total of 30 adult females with FFA were randomized to delgocitinib cream (n = 15) or cream vehicle (n = 15). After 12 weeks, expression of the T helper 1-related biomarker CXCL9 was significantly downregulated (-3.10; P < .05), whereas there were nonsignificant reductions in CXCL10 (-2.60; P < .1) and IFN-γ (-1.49; P = .22) in lesions treated with delgocitinib cream but not cream vehicle. Delgocitinib-treated lesions had a small but significant mean improvement in transcriptomic profile (4%; P < .001), whereas lesions treated with cream vehicle worsened (33%). Delgocitinib cream was well-tolerated and associated with improvements in exploratory clinical severity endpoints. LIMITATIONS: Limitations of the trial include small sample size, biomarker analyses only being conducted to week 12, and the exploratory nature of efficacy endpoints. CONCLUSION: Delgocitinib cream resulted in an improvement in the transcriptomic profile of lesions and may have potential as a topical treatment for FFA. This study is registered with NCT05332366.

OBJECTIVES: Inhibition of JAK-mediated signaling pathways leads to hair regrowth and improved quality of life in alopecia areata (AA) but may be associated with potential risks. This study assessed the benefit-risk profile of ritlecitinib, an oral selective JAK3/TEC family kinase inhibitor (JAK3/TEC), from the perspective of adult patients with AA to inform regulatory decisions. METHODS: We combined patient preference data from a discrete choice experiment involving 201 adults with AA in the United States and Europe, efficacy data for ritlecitinib 50 mg once daily and placebo from the ALLEGRO phase 2b/3 trial (NCT03732807) of ritlecitinib efficacy and safety, and safety data from an integrated safety analysis of 4 studies in AA. The discrete choice experiment included 6 treatment attributes: 3 benefits (chance of achieving ≥80% scalp hair coverage, moderate or normal eyebrows, and moderate or normal eyelashes) and 3 risks (3-year risks of serious infections, blood clots, and cancer). Choice probabilities were calculated using the estimated utility function. Weighted net benefit scores were calculated using multicriteria decision analysis. Stochastic multicriteria acceptability analysis assessed the impact of uncertainty and preference heterogeneity on net benefit conclusions. RESULTS: Ritlecitinib 50 mg had a higher weighted net benefit score than placebo with a predicted choice probability of 70.9% (95% CI 61.2%-77.5%), indicating that, on average, patients would likely evaluate benefits to outweigh risks. The probability of positive weighted net benefit based on the stochastic multicriteria acceptability analysis was 78%. BACKGROUND: Consuming only low-fiber food on the day before the procedure is recommended as standard practice for bowel cleansing in patients using polyethylene glycol (PEG). However, there have been few studies to date investigating the optimal dietary regimen for patients using the more cost-effective Sennoside A+B calcium before colonoscopy. The present study compares the quality of bowel preparation of patients following a clear diet and those following a low-fiber diet on the day before colonoscopy among patients using Sennoside A+B calcium. METHODS: This prospective, randomized, and investigator-blinded study was conducted between November 2023 and February 2024. Included in the study were 150 patients aged 18 years and over with a mean age of 55.6±14 years, with 75 patients assigned to the clear diet group and 75 patients to the low-fiber diet group. The assessment of colonoscopy cleanliness before colonoscopy was conducted using the Boston Bowel Preparation Scale (BBPS). RESULTS: None of the patients experienced intolerance. The mean overall total cleanliness score, measured using the BBPS, was 7.24±2.1, with 7.48 ± 2.1 in the low-fiber diet group and 7.0 ±2.1 in the clear liquid diet group. No significant difference in cleanliness was found between the 2 groups ( P =0.096). CONCLUSION: Patients using cost-effective Senna for bowel cleansing before colonoscopy can safely consume low-fiber foods on the day before the procedure. In the present study, a clear diet offered no benefit over a more comfortable and better-tolerated diet in terms of colon cleanliness.

BACKGROUND: The specific influence of whole gut transit time (WGTT) on microbiome dynamics and bile acid metabolism remains unclear, despite links between changes in WGTT and certain gastrointestinal disorders. Our investigation aimed to determine the impact of WGTT changes on the composition of the fecal microbiome and bile acid profile. METHODS: Healthy volunteers (n = 18) received loperamide, to decrease bowel movement frequency, and senna, a laxative, each over a 6-day period, in a randomized sequence, with a minimum 16-day interval between each treatment. Stool samples were analyzed for microbiome by shotgun sequencing and bile acid composition determined with high-performance liquid chromatography coupled to tandem mass spectrometry. Sera were examined for markers of bile acid synthesis. KEY RESULTS: Senna or loperamide decreased or increased WGTT, respectively. Treatment altered stool characteristics, bowel movement frequency, and stool weight. The senna-treated group had increased primary and secondary fecal bile acids; serum levels of fibroblast growth factor 19 were significantly reduced. Increasing WGTT with loperamide led to an increase in bile salt hydrolase genes, along with elevated bacterial species richness (p = 0.04). Thirty-six species exhibiting significant differences were identified, several of which have notable implications for gut health. WGTT displayed negative correlations with total primary (particularly chenodeoxycholic acid) and secondary bile acids (ursodeoxycholic acid and glycochenodeoxycholic acid). Treatment-induced changes in microbiome composition and bile acid metabolism reverted back to baseline within 16 days. CONCLUSION: Whole gut transit time changes significantly affect fecal microbiome composition and function, as well as bile acid composition and synthesis in healthy subjects. This consideration is likely to have long-term implications. © 2025 The Author(s). Neurogastroenterology & Motility published by John Wiley & Sons Ltd.

BACKGROUND: Etrasimod, an oral, selective sphingosine 1-phosphate 1, 4 and 5 receptor modulator approved for the treatment of ulcerative colitis, has been studied in immune-mediated inflammatory diseases, including alopecia areata (AA). OBJECTIVES: To evaluate the efficacy and safety of etrasimod in adults with moderate to severe AA. METHODS: This Phase 2, randomized, double-blind, placebo-controlled trial included patients (aged ≥18 years) with moderate to severe AA, defined as a Severity of Alopecia Tool (SALT) score of ≥25. Patients were sequentially enrolled into two cohorts. Cohort 1 included patients (SALT score of ≥50) randomized 2:1 to etrasimod 2 mg or placebo. Cohort 2 included patients (SALT score ≥25 to <95) randomized 4:1:2 to etrasimod 3 mg, 2 mg or placebo. Patients completed a 24-week double-blind and 28-week open-label extension period. The primary endpoint was percent change from baseline (%CFB) in SALT score at Week 24. Safety was monitored throughout the trial. RESULTS: Eighty patients were randomized to etrasimod 2 mg (n = 31), 3 mg (n = 25) or placebo (n = 24). At Week 24, least squares mean (SE) percent changes from baseline in SALT score for the etrasimod 2 mg, 3 mg and placebo groups were -13.8 (8.6), -21.4 (6.9) and 0.35 (8.9), respectively. The least squares mean difference (95% CI; P value) in SALT score %CFB of etrasimod 2 mg and 3 mg versus placebo was -14.1 (-38.9 to 10.6; p = 0.2579) and - 21.8 (-44.4 to 0.9; p = 0.0592), respectively; statistical superiority was not achieved. The proportions of patients achieving ≥30%, ≥50% or ≥75% improvement in baseline SALT score at Week 24 were generally numerically higher in etrasimod groups versus placebo. Treatment-emergent adverse events occurred in 67.7%, 80.0% and 78.3% of patients receiving etrasimod 2 mg, 3 mg and placebo, respectively, by Week 24. CONCLUSIONS: Etrasimod did not meet the primary and secondary efficacy endpoints, but efficacy was numerically higher with etrasimod than with placebo. The etrasimod clinical programme for AA has been discontinued. Etrasimod was well tolerated, and its safety profile was consistent with other etrasimod studies to date. TRIAL REGISTRATION: C

INTRODUCTION: No bowel preparation (BP) for colonoscopy achieves optimal efficacy and tolerability. Combining polyethylene glycol plus ascorbic acid (PEG-Asc) with adjuvants has been explored to enhance cleansing efficacy and reduce the required volume. The aim of this study was to evaluate whether adding 0.5 mg linaclotide to 1 L PEG-Asc (1 L-PEG/AL) improves superior cleansing compared with adding 24 mg senna (1 L-PEG/AS), which we previously reported to be noninferior to the standard regimen of 2 L PEG-Asc. METHODS: A multicenter, endoscopist-blinded, randomized controlled trial was conducted at 5 centers in Japan with outpatients scheduled for colonoscopy. The primary outcome was adequate BP, evaluated using the Boston Bowel Preparation Scale. Analyses were stratified based on low-risk and high-risk groups for inadequate BP. RESULTS: Between April 2022 and April 2023, 1,464 patients were randomized to the 1L-PEG/AL (n = 731) or 1L-PEG/AS (n = 733). The 1L-PEG/AL demonstrated higher adequate BP rates (92% vs 86%, P = 0.001) compared with 1L-PEG/AS. Both groups experienced similar adverse events and expressed high willingness for repeat BP (83% vs 81%, P = 0.49). In high-risk patients (n = 892), 1L-PEG/AL had significantly higher adequate BP rates (94% vs 86%, P < 0.001), whereas rates were comparably high in low-risk patients (n = 539) (94% vs 94%, P = 0.66). DISCUSSION: The linaclotide regimen was superior to the senna regimen reagrding BP efficacy without reducing tolerability. It can be a promising new option for BP, especially in patients at a high risk of inadequate BP.
